Coach who is also a Certified Youth Fitness Specialist through the IYCA.

the preparation for life.

SIGN-UP PROCEDURES
In order for you to reserve your spot into one of our Athletic Training Camps, you need to complete the following steps:
Step
1
If you
are just attending one of our 2-3 day Athletic Training Camps with The Works, you ONLY need forms A, B, & F.
a. Waiver, Release, and Assumption of Risk Form
b. Release of Liability
c. Peak Fitness
& Performance LLC Policies
d. Training Service Agreement
e. Physician's
Consent to Participate in a Fitness Program
f. Picture/Video Release Form
Step 2
Complete all forms accurately and legibly, and sign and initial all places where it is requested. Please
don't be intimidated by the forms...they're actually pretty simple to fill out. If you have any
question, please contact Peak Fitness & Performance LLC.
Step 3
Send all completed forms via US Mail to the following address below with
the first months payment either by personal check or money
order made payable to "Peak Fitness & Performance LLC".
You can also hand deliver the forms to the below address.

Step 4
Fill out
the form below stating that you have mailed the proper forms with the proper payment to reserve your spot in the camp. Also
please mention how old the athlete is so we can keep track of how many are in each group. You will be added to the list for
7 business days. If payment and forms do not arrive within 7 business days...you will be dropped from the list and contacted
by Peak Fitness & Performance LLC of the situation.
